Appointments

Appointments fill quickly.  Please schedule your annual exam 3-4 months before it is due.  Annual exams are for routine preventive care.  Problem-related visits will be scheduled as soon as possible based upon urgency.  Feel free to request an appointment online under Appointment Requests. 

 

Our appointment reminder system will call 3 days before your appointment to remind you of your scheduled time, so make sure that we have your correct telephone information.  Please give at least 24 hours notice if you must cancel an appointment so that we can use that time to accommodate other patients.

 

Visits

We want to make your visits go as smoothly as possible.  We recommend that you come to the office 15-20 minutes before your appointment.  To save time, complete or update the Online Registration.  Please bring your insurance card and a picture ID as we make copies of them at every visit so that your account is correctly billed. 

  • New Patients - Print and complete the appropriate new patient history forms.  If you have pertinent records from a previous doctor, please arrange to have a copy of your records sent to us.  You can print and complete the Authorization to Release Medical Records form and send it to your current physician. 
  • Established Patients - Prior to annual Well Woman visits please print the Established Patient History.

Telephone Calls

The telephone hours are 8-5 Monday-Thursday and 8-4 Friday. 

 

Please help us make the best use of your time and ours.  When leaving a message for the nurse, include your name, your doctor's name, your date of birth and briefly explain the reason for your call.  Always give us the best phone number to reach you.  Every effort is made to return calls by the end of the day.  If you have an urgent need after hours, call the main number and follow the prompts to contact the doctor on call. 

 

Established patients can also ask the nurse a question online by using the Ask a Question tab on the home page of this website.  Online questions will be answered during regular office hours.

 

Medication Refills

Processing refills through the pharmacy is the most efficient and accurate method.  Please call your pharmacy and they will fax us the complete information.  Patients can also request prescription refills online by using the Prescription Renewal tab on the home page of this website. 

 

If you need to call us about your prescriptions, (including birth control pills) please call during office hours.  These changes will only be made during office hours.  Please have the phone number for your pharmacy available at the time of your call.  

 

Allow 48 hours for refill requests. 

 

Business Office

If you have questions about your account, you may contact the business office by using the tab on the home page labeled Ask a Question.  If you want to call, follow the prompt for business office.  When no one is available, please leave a message with the best number to reach you.  Someone will call you back within 24 hours.

 

Release of Medical Records

Your medical record is confidential information. We will release your records, only with your written authorization in accordance with state and federal law.  The authorization form is available at Printable Forms on the column on the left.  Please print it, sign it and fax it to us at 210 946-1700, attention Medical Records.  Allow 5 working days for medical records to be available.

Our office is adjacent to the North Central Baptist Hospital.  To For more information please go to www.baptisthealthsystem.com/facilities_ncbh.aspx.  To learn about their women's and children's services go to www.baptistmomsandkids.com.  It will have information about childbirth classes under Events - Family and Parenting.  We encourage expectant moms and their families to attend the Maternal-Child Tours that are offered on the first Monday of every month at 5, 6 and 7 pm.
